Description: POSITION TITLE: Program Coordinator/Program Manager REPORTS TO: Director or Assistant Director SUPERVISES: Team Leader, Personal Assistants GENERAL RESPONSIBILITIES: Ensures quality, health and safety and client centered approaches for individuals in community living services. Supervises, supports and trains staff as they assist individuals served. Ensures compliance with federal, state and local regulations while expanding opportunities for persons served to participate in community life as they choose. SPECIFIC RESPONSIBILITIES: 1. Develop and implement the Individual Service Plan and assist in development of the Plan of Care for each person served. 2. Influencing and procuring Plan of Care that addresses all client needs and articulating elements to appropriate staff. 3. For assessment, planning, implementation, and review of person-centered support plans for people served. 4. Coordinate the delivery of all services provided by AWS and the community. 5. Assist people served in expanding their opportunities to participate in community life as they choose. 6. For providing on-call direct support and coverage (beeper) coordination. 7. Participate in the hiring, training and supervision of Team Leaders and Personal Assistants. 8. Schedule staff in accordance with Plan of Care and providing emergency coverage. 9. Accurate and timely completion of ISPs, Quarterlies, Addendums, client financials, QA walk throughs, monthly and daily documentation (logs, red, yellow and orange books, fire and tornado drills, client medical information, MARs, etc.) 10. Be aware of and submit reportable incidents within required time frames. 11. Manage and monitor client finances within required time frames (including ICLBs, checkbooks, RLAs, client financial profiles, etc.) 12. Serve as liaison with external case managers, families, other agencies and internal programs. 13. Work collaboratively with director, nurse, HR, accounting, IT, etc. 14. Perform other duties as assigned. 15. Comply with all standards to assure the health and safety of all staff and clients we serve. QUALIFICATIONS: 1. Bachelor's degree in human services and one (1) year experience working with developmentally disabled people or five (5) years experience 2. Valid driver’s license 3. Direct Support Professional

Description: POSITION TITLE: Direct Support Professional Supported Living DSP: Reports to Program Coordinator (PC) and Supported Living Manager (SLM) SUPERVISES: No Supervisory Capacity GENERAL RESPONSIBILITIES: Provides support and assistance to people served who have developmental disabilities. Assists people served in expanding their opportunities to participate in community life as they choose. Assists individuals in learning and maintaining skills and having new life experiences. Provides training to persons served in all identified areas and function as part of the interdisciplinary team. Must be willing to flex schedule according to the needs of the individual(s) served. SPECIFIC RESPONSIBILITIES: 1. Participate proactively as part of an interdisciplinary team in the development and implementation of each served person's individual plan to achieve identified outcomes. 2. Ensure the individual's health and safety while pursuing community opportunities and at home (or for Day Program, in the classroom). 3. Assist the individual served to acquire, retain or improve skills as identified in the person's ISP. 4. Assist the person served with daily living skills, including but not limited to personal care and household tasks (meal preparation, cleaning, laundry, etc.). 5. Maintain a clean and orderly environment for person(s) served. 6. Practice/participate in safety drills as scheduled. 7. Assist the person served in accessing community resources, shopping, and recreational activities. 8. Administer medications and follow physicians' orders concerning medications and treatments. Assist the nurse in meeting the served individual's healthcare needs. 9. Monitor the general well-being of persons served and follow supervisor/nurse instructions, including all healthcare appointments. 10. Communicate to nurse and supervisor health issues and concerns as they arise regarding individuals served. 11. Provide communication to supervisor(s) regarding persons served, home, day program or work related needs (as applicable to each individual). 12. Comply with and implement all protocols for individuals including, but not limited to: high risk plans, dining plans, seizure management plans, positioning schedules, etc. (as applicable to each individual). 13. Follow food management policies and assist persons served in preparing nutritious meals according to their dining plan. 14. Complete proper documentation relating to the served persons' healthcare, including MARs, doctor visit forms, healthcare notes, etc. 15. Complete all required programmatic documentation accurately and on time, including but not limited to daily documentation, injury/illness reports, ISP data sheets, notable occurrences, etc. as required by AWS, state, and federal regulations. 16. Attend required trainings and meetings, (i.e. staff meetings, house meetings, etc.). 17. Report any suspected abuse, neglect or exploitation immediately to supervisor or department head. Maintain employment-required documentation (current auto insurance, driver's license), certification (i.e., First Aid, CPR, Medication Administration, Mandt, etc.) and training (OSHA, Abuse & Neglect, etc.) as required to comply with federal, state and program requirements and standards. 21. Comply with all standards to assure the health and safety of all staff and the persons we serve. 22. Safely transport individuals served as required. 23. Plan and participate directly in age-appropriate recreational, therapeutic and training activities of the persons served. 24. Meet consumer service/billable time goals. 25. Perform other duties as assigned. ADDITIONAL REQUIREMENTS FOR DSPs WORKING WITH INDIVIDUALS WHO HAVE A BEHAVIOR SUPPORT PLAN (BSP): 1. Comply with behavior support policies and procedures, and implement approved BSP. 2. Provide positive behavior supports according to individual BSP's and respond to emergency situations as trained. ADDITIONAL REQUIREMENTS FOR DSPs WORKING IN ICF MR FACILITIES: 1.
