Description: GENERAL RESPONSIBILITIES: Provides support and assistance to people served with developmental disabilities in their own home. Assists people served in expanding their opportunities to participate in community life as they choose. Assists people in learning new skills and having new life experiences. SPECIFIC RESPONSIBILITIES: Insure the individual’s health and safety while pursuing community opportunities for personal growth. Work proactively to achieve outcomes identified in the individual’s ISP. Assist the person served in personal and household maintenance. Assist the person served in accessing community resources, shopping, and recreational activities. Administer medication and assist the nurse in meeting client’s health care needs. Assist the person served to acquire, retain or improve skills as identified in the ISP. Complete necessary documentation accurately and on time (ex., data tracker, red books, med sheets goal sheets, etc.). Attend required meetings, i.e., house meetings, staff meetings, client ISP meetings, etc. Must report any suspected abuse, neglect or exploitation immediately to supervisor or department head. Maintain documentation, certification and training as required to comply with federal, state and program requirements and standards. Perform other duties as assigned. Comply with all standards to assure the health and safety of all staff and clients we serve. Requirements: 1. Must have Valid driver's license. No major issues within the past 3 years. 2. Experience working with people with developmental disabilities preferred. 3. Must have high school diploma or equivalent. 4. Must be able to follow oral or written instructions and complete required documentation. 5. Must be at least eighteen (18) years of age. 6. Description: POSITION TITLE: Program Coordinator/Program Manager REPORTS TO: Director or Assistant Director SUPERVISES: Team Leader, Personal Assistants GENERAL RESPONSIBILITIES: Ensures quality, health and safety and client centered approaches for individuals in community living services. Supervises, supports and trains staff as they assist individuals served. Ensures compliance with federal, state and local regulations while expanding opportunities for persons served to participate in community life as they choose. SPECIFIC RESPONSIBILITIES: 1. Develop and implement the Individual Service Plan and assist in development of the Plan of Care for each person served. 2. Influencing and procuring Plan of Care that addresses all client needs and articulating elements to appropriate staff. 3. For assessment, planning, implementation, and review of person-centered support plans for people served. 4. Coordinate the delivery of all services provided by AWS and the community. 5. Assist people served in expanding their opportunities to participate in community life as they choose. 6. For providing on-call direct support and coverage (beeper) coordination. 7. Participate in the hiring, training and supervision of Team Leaders and Personal Assistants. 8. Schedule staff in accordance with Plan of Care and providing emergency coverage. 9. Accurate and timely completion of ISPs, Quarterlies, Addendums, client financials, QA walk throughs, monthly and daily documentation (logs, red, yellow and orange books, fire and tornado drills, client medical information, MARs, etc.) 10. Be aware of and submit reportable incidents within required time frames. 11. Manage and monitor client finances within required time frames (including ICLBs, checkbooks, RLAs, client financial profiles, etc.) 12. Serve as liaison with external case managers, families, other agencies and internal programs. 13. Work collaboratively with director, nurse, HR, accounting, IT, etc. 14. Perform other duties as assigned. 15. Comply with all standards to assure the health and safety of all staff and clients we serve. QUALIFICATIONS: 1. Bachelor's degree in human services and one (1) year experience working with developmentally disabled people or five (5) years experience 2. Valid drivers license 3.
