Description:

GENERAL RESPONSIBILITIES: Performs all the functions of Direct Care and assists the Program Coordinator in supervising, supporting and training Personal Assistants and in ensuring quality of services and the health and safety of the individual served.

SPECIFIC RESPONSIBILITIES:

  1. Collect and disseminate forms and agency information.

  2. Assist with training and monitoring Personal Assistants/Trainers.

  3. Help develop and manage staffing schedule.

  4. Responsible for ensuring that the individual’s home is safe, clean and in good repair.

  5. Help collect and manage individual’s financial information.

  6. If applicable, assist with and general budget and money management.

  7. Assist in monitoring individual’s health care needs

  8. Monitor proper diet, meal planning and shopping.

  9. Ensure that monthly safety drills are conducted and documented.

  10. Must report any suspected abuse, neglect or exploitation immediately to supervisor.

11. Maintain documentation, certification and training as required.

  1. Perform other duties as assigned.

  2. Comply with all standards to assure the health and safety of all staff and clients we serve.

ASSIST WITH THE FOLLOWING:

*Review of goals and preparation of goal sheets

*Planning and follow-through of doctor appointments

*Updating client files

*Planning living arrangements for clients

 *Assisting in crisis situations

*Meeting with diagnostic and evaluation team

*Review and checking in medications

*Assist with medical emergencies

*Assist with quarterly reports

*Assist with coordinating staffing coverage

 *Train new employees

*Complete home observations

QUALIFICATIONS:

1. Valid driver’s license

2. Experience working with people with developmental disabilities

  1. High school diploma or equivalent

  2. Must be at least eighteen (18) years of age
